I think this is an old model hybrid from Fourteen. Does anyone recognize it and perhaps provide further details. Does this have a .370 hosel?

Yes SYB's designs have been closely related in the past with Fourteens.

This was a retail SYB club released around the time of the first Fourteen driving irons.

SYB is a component company and this club would be worth anywhere from 50-75 bucks with a DGS300 shaft.

I am pretty sure it is a .370 hosel.
